Form 80 (Personal Particulars for Character Assessment) is a mandatory form required by the Department of Home Affairs from most adult visa applicants. It collects personal history information that allows the Department to conduct a thorough character assessment, including criminal history checks, travel history, and background on prior residences and employment.

Form 80 must be completed honestly and in full — providing false or misleading information on Form 80 is a basis for refusal under PIC 4020 (misrepresentation) and may attract criminal liability.

Form 80 is prescribed under the Migration Regulations 1994. The requirement to complete it is specified in the Schedule 1 entry for each visa class that requires it. Not all visa classes require Form 80 — the specific requirement is listed in the Schedule 1 applicant criteria for each visa.

Form 80 requires the applicant to disclose:

• Personal details — full name, date of birth, all passports held (current and previous)
• All countries resided in for 12 months or more (over the past 10 years)
• Employment history (over the past 10 years)
• Military service history
• Membership of any organisation (political, military, quasi-military, terrorist, or criminal)
• Criminal convictions in any country (including spent convictions)
• Whether the applicant has been charged with or convicted of any offence involving violence, drugs, human trafficking, terrorism, or fraud
• Whether the applicant has ever been refused a visa to any country, or removed/deported from any country

Completed Form 80 enables the Department to conduct Police Clearance Certificate (PCC) checks and National Police Check (NPC) requests in relevant countries. For some visa classes, the Department requests PCCs directly; for others, applicants must obtain them independently.

A common mistake by applicants is to leave questions blank or to answer "N/A" where they have genuinely nothing to disclose, rather than "No." This can raise concerns about completeness. Form 80 should be completed with explicit "No" responses where no history exists, rather than blank fields.

The form is available on the Department's website and is typically submitted as an attachment to the ImmiAccount visa application. It is generally required to be submitted with the initial application, not at a later stage. Failure to submit Form 80 when required may result in the application being considered invalid or incomplete.
